    A mother of three was shot dead, her husband was injured, and her ex-boyfriend killed himself— at a central Pennsylvania home with her children inside, authorities say. Joseph Paul Pentz, 32, of Etters, shot Jessica McCulloch, 36, of New Cumberland, dead at her home in the 1200 block of Bridge Street in New Cumberland around 6:15 a.m. on Wednesday, Apr. 6, according to the Cumberland County coroner's office. Both died from gunshot wounds to the head, the coroner's office says. McCulloch's husband was also injured and remains in the hospital, PennLive reports. It is unclear when Pentz and McCulloch broke up, as Pentz still states they are in a relationship on Facebook and both have photos of them up on their pages.     The off-duty Illinois State Police trooper and his wife who were found shot dead inside a parked car on Chicago's South Side Monday have been confirmed as a murder-suicide.   The family of Amanda Alvarez-Calo, 31, said that she was on the verge of leaving her husband, 30-year-old Antonio Alvarez, at the time of her death.  Chicago police said that at around 1.40pm on Monday, a witness came across a car in the 11000 block of South Avenue E and discovered inside the bodies of Trooper Alvarez and Alvarez-Calo. The couple were pronounced dead at the scene from gunshot wounds to the head,  state police said.     A Texas man allegedly killed his wife and then turned the gun on himself in a Christmas morning murder-suicide that left their five young children orphaned. Nurse Kristen Slack, 45, from Universal City just outside of San Antonio was killed on Saturday morning by her boyfriend Todd Danhof having been shot multiple times.     RICHMOND — A Richmond father killed the mother of his children, then turned the gun on himself inside the couple’s residence, police said. The murder-suicide happened Tuesday morning in the 700 block of Thomas Drive, Richmond police Sgt. Aaron Pomeroy said. Police were called to the residence around 9 a.m. and found the two in a back bedroom. The Contra Costa County Coroner’s Office on Friday identified the woman as 28-year-old Claudia Justinian Mendoza, of Richmond. She had been shot several times, Pomeroy said, and died at the scene. Her husband, identified as 33-year-old Erick Magallon Izquierdo, had a single gunshot wound, Pomeroy said. According to police, the two were experiencing marital problems and in the process of separating. They have  three children, ages 4-12, Pomeroy said. They were not home when the shootings happened.
